I'd recommend a FAQ entry which would take care of most of the OLC ones:

Q: Why does my OasisOLC v1.5 or greater crash when creating a new mob?

A: Until proven wrong, the problem is not with OasisOLC.  You have a memory
   overrun somewhere in your code.  Check for sprintf or strcpy statements,
   especially those in a loop.  Note that a few of the problems in the past
   were actually in stock CircleMUD although all known problems have been
   fixed.  Those problems were with referring to array index -1.  If you do
   not have OasisOLC v1.3, upgrade.
