For a _good_ list of various medieval style awards go to www.sca.org, hit
the Geography section and then head to the main pages of each of the 14(?)
Kingdoms and the various Principalities. On those main pages you should
find links to awards, and you can see what kind of awards are given, what
they are called, who awards them, who receives them, how common they are,
and why they are given.

I would add a struct similar to the PRF_* or PLR_* and use bitvectors.
Yes, some awards can be given more than once, but for the most part awards
are either given (ON) or not given (OFF).

I've been contemplating something similar, but not quite sure how I'm
going to do it just yet.
